The info below is pretty much the way I did it when I sent my Hasbro sabers in for conversion:
Whenever you get your price quote, you would need to go to the below thread and use the requisition charge (each time you click on it, it charges you $1.00) for the cost they tell you it will be. You then will proceed to the checkout section where you will select what kind/how fast you want it shipped back to you. Don't forget to tell them what you want done in the special instructions window before submitting the purchase.

Make sure you package the saber well (hopefully you saved some of the packaging peanuts), and include a copy of any emails between you and US, along with a copy of the document from the transaction you accomplished with US from the first paragraph.

Take the package to _____ (I recommend UPS since US uses them, but I have used FedEx) and pay for the shipping. The shippers who you went with can tell you when the package is estimated to be there.
Lastly, not really necessary, but as a courtesy, I forward an email to US (with all the email conversations attached) to let them know that I sent the package to them, and what date it is estimated to be there.
Shuuuu, that was a long answer. Another thought, and possibly cheaper, do you know anyone who is gifted in electronics who could put on a pair of quick disconnects? This would save you the shipping charges. But if you are like me, I would have to go the US route myself.
